APS

All applicants with an active Adult Protective Service (APS) case are placed on the APS waitlist. The current wait time is maximum 30 days. CLICK HERE for steps to apply. 

SNF/Hospital

All persons admitted to a skilled nursing facility (SNF) or hospital are eligible to be assessed for the assisted living waiver, once admitted for 45 days or more. * once assessed patients/residents may not discharge from the facility until their 61st day of admission.
