Who is eligible?
Individuals with an active adult protective service case (must provide a formal referral from your case worker on county letterhead)
Individuals with active Medi-Cal
Individuals with the ability to pay rental fees
Individuals who can either make their own decisions or have a written authorized representative to make decisions on their behalf
Designated Power of Attorney (DPOA)
Advance Healthcare Directive (AHCD)
State Conservator
Complete the required Community Intake Packet.
Return the documents to our office via email or fax.
Get a recommendation for RCFE (LIC602A) from your primary care physician.
Instruct your APS case worker to send a letter to our office.*
•Letter must be on county letterhead stating there is an open case with the case number.
Once received, you will be placed on the APS waitlist

If the resident is currently a resident at an assisted living waiver, download the LIC603 document and provide it to the facility to be completed.
