The California Department of Public Health (CDPH) invites registered P21 2.0 attendees to participate in the Meeting Poster Session during lunch on Wednesday May 17, 2017.
POSTER SUBMISSIONS: provide an opportunity for you to share success stories, new data, innovations in programs and policies, or other relevant information about your organization’s work related to chronic disease prevention.

Abstracts for the Poster Session are due by Monday, April 17th, 2017.
ABSTRACT FOR POSTER SUBMISSION NOTIFICATION:
Corresponding authors will be notified via email of abstract status by Thursday, April 27, 2017.
RULES FOR SUBMITTING AN ABSTRACT FOR THE POSTER SESSION:
Please note that CME Credit is not offered for the Poster Session.
1. HEALTHY COMMUNITIES: These abstracts should report on community conditions and determinants of health; land use, planning, and health equity; and/or how community and clinical systems can work together to address social determinants to improve health.
2. OPTIMAL HEALTH SYSTEMS LINKED WITH COMMUNITY PREVENTION: These abstracts should report on community clinical partnerships for prevention; and/or prevention strategies and models that work.
3. ACCESSIBLE AND USABLE HEALTH INFORMATION: These abstracts should describe use of technology to facilitate data collection and surveillance; and/or creation of new partners in data collection, surveillance and program evaluation.
4. PREVENTION SUSTAINABILITY AND CAPACITY: These abstracts should describe 1) the value of prevention funding on improving community health outcomes, 2) the opportunities for engaging in statewide prevention funding innovations, and/or 3) the challenges encountered when creating statewide funding streams.
It is expected that all conference poster presenters will disclose any significant financial interests or other relationships with any organization, manufacturer of commercial products, and/or provider of commercial services discussed in their presentations. The intent of this disclosure is not to prevent individuals with significant financial or other relationships from displaying posters, but rather to provide conference attendees with information on which they can make their own judgments.
