The California Department of Public Health (CDPH), in partnership with internal and external state public and private stakeholders, will convene leaders from multiple sectors and disciplines to inspire continued collaborative and synergistic work to achieve a broad, coordinated chronic disease movement.
The intended audience includes leaders from local public health departments, local, regional and state non-profit community partners, health providers, government agencies, academic institutions, voluntary, advocacy and policy organizations, foundations, and others committed to chronic disease prevention.
An evening reception followed by a one-day statewide meeting that features presentations on best practices and innovative solutions, and provides opportunities for networking and for collective action to further chronic disease prevention and health in California. The conference will be participatory and interactive.
The California Wellness Plan - California’s Chronic Disease Prevention and Health Promotion Plan - is intended to provide a roadmap to eliminate preventable chronic disease deaths, reduce the number of Californians living with chronic disease, improve the quality of life of those with chronic disease, lower the cost of care, and achieve health equity.

Participants will have the opportunity to hear plenary presentations on each goal, as well as select from one of eight goal-specific breakout sessions.
