The PGME office and AHS also have forms that need to be completed at least 3 months prior to an outside elective. 

For PGME, residents please visit their page for details and complete the appropriate form, then send to the PA to upload to SharePoint: https://cumming.ucalgary.ca/pgme/current-trainees/outgoing-electives

Evaluation process for Out of Town Electives: 

EPAs are able to be completed by preceptors during your out of town electives. There are two options, either the EPA can be pulled and made into a fillable PDF form, or the resident can trigger an EPA with the PA as the evaluator, then print the blank EPA and have the preceptor complete by hand. Once the preceptor has completed the observation, it must be returned to the PA to enter into MedSIS.
