End of Rotation Feedback forms will be sent to the last preceptor you were with (i.e. the preceptor who spent the majority of the last week of the block with you).

The Clinical Teacher Evaluation form will also be sent to you for completion at the end of each rotation. Please be aware that these forms are gathered together and distributed cumulatively with all learners (Gen Peds residents, electives, other subspecialty residents) to each Preceptor at the end of each year. They will not have any identifying information and will only be sent to the preceptor if a minimum of 3 evaluations are received.

The Rotation Evaluation form is also to be completed at the end of each rotation. These are analyzed on a yearly basis and are used to make improvements to each rotation.

These 3 evaluations will be automatically sent near the end of each rotation through MedSIS.
