Emergency Care Plans

If your child has been diagnosed with a life threatening medical condition (i.e.: asthma, severe allergy, cardiac history, diabetes, epilepsy/seizures), we need forms to be filled out in order to assure your child is safe while at school, and to meet state requirements. The forms need to be completed BEFORE starting school every year.

There must be:

1. Physician orders for medication or treatment,

2. Medication available at school, &

3. An Emergency Care Plan filled out by your child’s doctor - which will assist school personnel to know exactly how to help your child in the event of a life threatening event (May get specified Emergency Care Plan from the school nurse).

If we do not have the items listed above we are unable to safely care for your child while in school, and your child will not be able to start school until we have the paperwork listed above.


Complete these steps in order to meet the requirements:

1. Fill out and sign all the needed Emergency Care Plan forms for your child’s medical condition.

2. Make an appointment with your child’s health care provider. Please have the provider fill out any prescriptions, and fill out the “Healthcare Provider” section of the Medical Authorization/Med permit form.

3. Bring all COMPLETED paperwork and medications (in their original bottles) to school nurse. This should include:

· Updated information including medical authorization signed by your child’s healthcare provider.

· Appropriate medication and supplies depending on your child’s condition:

o For Bee sting/Food/Nut Allergy: an Epi-Pen and/or Benadryl

o For Asthma: Albuterol inhaler and/or nebulizer solution and machine

o For Diabetes: glucose tablets, glucagon, subcutaneous needles, lancets, ketone strips, and extra batteries for the glucose monitor.

o For Epilepsy/Seizures: any medication that would need to be given

· If life-saving medication is ordered from your child’s healthcare provider, the school will need an order for it to also be kept at school.

· If life-saving medication or an Emergency Care Plan are no longer needed, please submit a doctor’s note.

4. Plan on meeting with the school nurse personally if you do not mail or drop off the forms, and medication before school starts.
