Student and Parent(s)/Guardian(s) may opt to remove grades from the transcript or request a passing (non-numerical) grade which does not impact a student’s GPA. Please sign and have your parent sign this form as well. Return to your counselor once you have completed these steps.

Policy 2410: Students may receive a maximum of 1.0 (1 season = 0.5 credits) credit toward the Physical Education requirement for participation in a community based sport that is not offered by the district or for participation in a WIAA approved sport. 

Students must have already passed a 0.5 High School Physical Education prior to requesting credit for athletics.
