• For physicians in private practice, write "N/A" in the immediate supervisor field

   • File must be labelled with SURNAME, FIRST NAME, MIDDLE INITIAL

B. FOR SELECT REGISTRANTS

Additional Documents: 

• For college undergraduates who are presently working:

Proof of 2 years minimum work experience in OHS (e.g Recommendation Letter from the current/previous Employer or Certificate of Employment proving that you have worked/is working in the OHS field)

• For unemployed professionals, those who have completed a bachelor's degree, or graduates who are not working in the field of OHS at the moment:
