Acknowledgement:
This form must be submitted with your summons attached within 48 hours of receipt.
A clear picture or a scanned copy of the summons will be accepted.
You will be required to provide adequate proof of jury duty service.
Lineholders will receive flight pay credit and all accruals for any flight time lost due to the requirement to be present for Jury Duty.
Reserves will receive pay credit at the daily rate and all accruals for each reserve day lost due to the requirement to be present for jury duty.
Any changes made to your schedule after jury duty notification will not be honored for pay protection.
You are subject to reassignment on those days originally scheduled for jury duty, which do not conflict with the jury duty assignment. Please note that if you are reassigned and your jury duty is extended, you must notify your supervisor of your inability to fly.
After five or more consecutive days of jury duty, or combination of flight duty and jury duty, you will be guaranteed two additional days off with no loss of pay.
You must call the Flight Attendant Support Team at FAST at 1-800-FLT-LINE, Options 1 then 2 prior to any pairing(s) or reserve day(s) so that your schedule can be adjusted.
Upon release from your entire jury duty obligation, you must call the crew desk at FLTLINE option 4 then option 2 for reassignment.
You will be required to submit a record of service once completed. The record can either be uploaded to your case on Help Hub or submitted to your supervisor. A clear picture or a scanned copy of the record will be accepted.Β
If you are selected for trial or your jury duty is extended, please upload supporting documentation to your case on Help Hub. If you are not provided documentation from the Court, please contact your supervisor.

A Flight Attendant who is called upon to perform jury duty shall submit a Jury Duty request on Help Hub and/or notify her/his supervisor no later than forty-eight hours after receiving such notice. Your summons must be attached to the Help Hub form. A clear picture or a scanned copy of the summons will be accepted.
You must call the Flight Attendant Support Team (FAST) at 1-800-FLT-LINE, Option 1 then Option 2 prior to any pairing(s) or reserve day(s) so that your schedule can be adjusted to reflect 'jury duty'.
Upon release from your entire jury duty obligation, you must call the crew desk at 1-800-FLT-LINE, Option 4 then Option 2 for reassignment.

If you are provided documentation from the court for this extension: upload the documentation to your existing Jury Duty case and advise of your new ending service date. Your work history will be updated accordingly.
If you are not provided documentation from the court for this extension: contact your supervisor. Your supervisor will need to submit your extension on your behalf.Β
Β Β Β Β Β Β Β If you do not advise the Company of your jury duty extension, you will be placed on a Missed Trip until you contact your supervisor
Missed Trip:
A flight attendant who does not submit summons prior to calling in for Jury Duty will be placed on a Missed Trip.
A flight attendant wishing to change this MT to Paid Jury Duty must submit summons to his/her assigned supervisor. The supervisor must then submit a Jury Duty Help Hub form to advise the Company to change the schedule.
Pay:

If you adjust your schedule after receiving a jury duty notice, you will only be entitled to receive flight pass loss, or pay for missed reserve days, in your original schedule.

Commuting flight attendants are able to request to be released from a reassignable day on the day before they are scheduled to attend jury duty service if they are concerned about being in place for jury duty. Commuters, please request this when calling FAST to update your schedule.
If you are a commuter and you were reassigned a trip which conflicts with your ability to be in place for jury duty, please contact your supervisor. Your supervisor will determine if the conflict is legitimate and, if so, will advise FAST to drop the trip as unpaid. A flight attendant cannot request to have the trip dropped as unpaid. This must be requested by the supervisor.

Subpoena/Court Witness Summons
A subpoena or witness summons is an order issued by the court. The subpoena usually requires you to appear at a certain place, date, and time to testify as a witness about a particular case. In a criminal case, you can be subpoenaed only to testify in court. In a civil case, you may be subpoenaed for out-of-court testimony as well.

COMPANY RELATED MATTERS:
If a flight attendant receives a subpoena or witness summons to testify on behalf of United Airlines,Β Β
A Lineholder Flight Attendant who is called upon to appear in court as a witness for the Company shall receive flight pay credit, and all accruals, for any flight time lost due to the requirement to appear as a witness for the Company.
A Reserve Flight Attendant shall receive pay credit at the daily rate, and all accruals for each reserve day lost due to the requirement to appear in court as a witness for the Company.
A Flight Attendant who adjusts her/his schedule after receiving a notice of obligation to appear in court as a witness for the Company shall only be entitled to receive flight pay loss, or pay for missed reserve days, in her/his original schedule. The Flight Attendant who appears in court as a witness for the Company will be reimbursed for reasonable actual expenses incurred.
PERSONAL MATTERS/ Unrelated to United Airlines cases:
If a flight attendant receives a subpoena or witness summons to testify for personal or matters not related to a United Airlines case, flight attendants should first try to modifyΒ their own schedule updates to make themselves available on the day(s) they are required to appear.Β Flight attendants should utilize trades, PTO, PD and if the FA is still unable to modify their schedule. A Flight Attendant who is required to appear in court as the result of being subpoenaed or being named party in a court action shall be excused without pay from regular duties on days when she/he is required to be present.
Flight Attendant shall be allowed a reasonable amount of time to travel between her/his home and her/his Base in order to complete such service.
A Flight Attendant may be required to provide adequate proof of such service.
How do I inform the Company of my jury duty?
You can either submit the Jury Duty form on Help Hub or notify your supervisor who will submit the form on your behalf.
I have submitted my jury duty summons; do I need to do anything else?
Yes. Once you have submitted your summons, you will need to call FAST at 1-800-FLT-LINE, Options 1 then 2 prior to any pairing(s) or reserve day(s) to update your schedule. Once your entire jury duty obligation is complete, you will need to call the crew desk for reassignment.
Do I need to show proof of service?
Yes. If you submitted a Jury Duty Help Hub form, you can either upload record of service onto your case in Help Hub or submit it to your supervisor. If you did not submit a Help Hub form, you must submit record to your supervisor.
I was not paid for Jury Duty because I didn't submit my summons. How do I receive the correct pay?
If you do not have the proper Jury Duty Authority in your Work History when calling in for Jury Duty, your schedule will be actioned with a Missed Trip. You will need to submit your summons to your supervisor and they will then submit a Jury Duty Help Hub request to have your schedule changed to Paid Jury Duty
