Pre-Departure Paperwork

Before you commit to the program

• Review program dates, fees, and academics to make sure that the program will work for you personally, financially, and academically.

• Log into your application and submit the items listed on the Priority Checklist 

• Once you commit to the program, a $400 non-refundable deposit will be billed to your student account, and you will be held to the cancellation policy.

Log into your application account and submit the following paperwork via your checklists: 

• Academic Information Form

• Arrival Information Form

• Digital Photo

• Emergency Contacts

• GAPS Health Module (for Public Health theme students)

• Global Identity Course Form

• Health and Safety Orientation (parts 1 and 2)

• Housing Preference Form

• ISDSI Pre-departure Forms

• MSID Letter of Introduction

• MSID Research Proposal* (if applicable)

• MSID Thailand Academic Information

• MSID Thailand Background Check

• MSID Thailand Visa

• Passport Information (If you do not have a passport yet, apply for one right away and upload it as soon as you receive it.)

• Release & Waiver 

• Smart Traveler Enrollment Program Registration

• Transcript Order Agreement* (non-UofM students only)
