Confirmation Paperwork

To Confirm Your Place in the Program 

• Review program dates, fees, and academics to make sure that the program will work for you personally, financially, and academically.

• Log into your application and submit the items listed on the Confirmation Checklist 

• Once you are confirmed on the program, a $400 non-refundable deposit will be billed to your student account, and you will be held to the cancellation policy.

Confirmation Paperwork

Log into your application and submit the following paperwork: 

• Release & Waiver 

• Transcript Request Form* (non-UofM students only)

• Health Information Form

• Passport Copy (If you do not have a passport yet, apply for one right away and upload it as soon as you receive it.)

• Housing Form

• Orientation—Health and Safety (The enrollment key is: lacabroad. Find the orientation and directions on your checklist. This will be checked off of your checklist within one week of completion.)

•  Academic Information

• Research Proposal (if applicable)

• Letter of Introduction

• Digital Photo

• GAPS Module (for Public Health track students)
