Anonymous Feedback Form: Why we encourage feedback

Sometimes things simply do not go right. Core team is committed to ensuring that New Movement remains an inclusive and welcoming space for all dancers. We make mistakes, as do our members, but we will do our best to address and correct these mistakes. The form does not collect any identifying information unless you add your name.

Reasons you might use this form:

• Suggestions for improvement

• Feeling excluded

• Another dancer/group of dancers is creating a hostile environment (include as much detail as comfortable)

• Experiencing discriminatory or exclusionary actions by another member
