Henry Ford should respond to you within 5 days to help set-up an appointment. You don't have to be a Henry Ford patient but they do only serve the areas around their health care system.

Logisticare

BOOK: Members, families, social workers, and health care professionals all book rides via our online reservation portals or call centers. LogistiCare ensures eligibility and schedules the most appropriate transportation provider to meet their needs.

RIDE: Drivers pick members up at their homes, take them to their appointments and bring them home in a timely manner. If that ride doesn’t happen to the member’s satisfaction, we care, and will assign a staff member to help. More often than not drivers go above and beyond the call of duty.

MONITOR: LogistiCare monitors the ride as it is taking place via a number of safety measures, including GPS tracking and random curbside checks by field monitors. Good quality means ensuring the ride happens as intended. Vigilant monitoring is key to that effort.

SMART Bus

Info: SMART, in partnership with the Macomb County Health Department is assisting in registering residents who qualify on a wait list. To qualify for the wait list, you must meet the County requirements for age and currently live in Macomb County. The County will use the wait list to contact YOU to set up appointments when vaccines are available. SMART will also make free transportation arrangements for those that request it when scheduling the vaccine appointment.

GoGo Grandparent

Info: GoGoGrandparent allows older adults to utilize on demand transportation companies like Lyft while keeping family and loved ones notified.

WOTA

Info: WOTA is offering free rides to our residents (Western Oakland County) to get their vaccines within our service area! The even better news? Any of our residents 18+ or older is eligible to be taken to their vaccine appointment. All riders must be registered and must make their appointment 48 hours in advance.

NOTA is a transportation service serving seniors, disabled, and low income residents of Oxford, Orion, and Addison Townships, and the Villages of Oxford, Orion, and Leonard since 2001. NOTA operates 16 routes Monday to Friday. Saturday and Sunday NOTA operates two routes for local services only.

The residents NOTA serves are transit dependent individuals who need help getting to medical appointments, grocery shopping, community events and work. Without NOTA many riders would not be able to leave their homes or live an independent life.

We operate seven days a week and have 22 vehicles and 21 are wheelchair equipped.

AgeWell Services

Info: We provide medical transportation in the Muskegon County in Michigan.


Eligibility Requirements

You must be 60 years or older

You must be at 150% of the poverty level or less

Lakeshore Ride

Serving Muskegon, Ottawa, Allegan and Kent Counties

Info: Non-emergency medical and non-medical transportation for any ambulatory client or client who can transfer from a wheelchair. Same day transportation, door-to-door on request.

Fees/Fares: $5.30 for the first mile, $1.80 per mile thereafter

RideLink

Info: The Rapid participates in RideLink, a network of area transportation providers. RideLink offers transportation to persons aged 60 or older to any destination, including doctor visits, recreational visits, and shopping trips within Kent County.

RideLink passengers are encouraged to make a $2.00 donation per trip. Donations are an important part of this program. The money goes directly back into the program to allow for continued service.

Canton Twp Fire Department

Eligibility: Plymouth/Canton Residents 16+

Info: Our Firefighter/Paramedics are now approved COVID vaccination providers. This week, our Firefighters delivered COVID-19 vaccines to some of our community's most vulnerable and homebound residents.


Wayne County is holding vaccination clinics for Plymouth and Canton residents, 16 and older, in the weeks ahead.


Nurses on the Run

Eligibility: Washtenaw County Only


Info: Nurse on the Run provides vaccination in small settings such as group homes or assisted living. Nurse on the Run can also provide vaccination to individuals in their homes. Capacity is limited and individuals must meet the definition of homebound.
