MI-AORTA Outreach APP and Referral Liaison
We would like to introduce you to Allison DiBrano, a Nurse Practitioner in our MI-AORTA program. She is the first point of contact for patients in our program. Allison works in collaboration with our specialist to coordinate referral and clinic visits, helping to provide safe and effective system-wide coordination of care. The MI-AORTA clinic allows Allison to see patients within one week of referral. This means patients will get quicker access to a provider and a customized plan of care.
When is it appropriate to refer to the MI-AORTA program?
The MI-AORTA Program can assist in the management of aortic disease in the following scenarios:
When a patient requires access to a cardiologist or vascular medicine specialist.
This could involve assistance with appropriate image ordering and surveillance, risk factor modification, medication initiation and adjustment, evaluation of family history with potential need for genetic testing/surveillance, and other related services.
When a patient with aortic disease requires access to a cardiac surgeon.
This may involve urgent or routine surgical evaluation, pre-operative testing/work-up, explanation of different surgical procedures/options for referred diagnosis, appropriate follow-up in regards to image surveillance, post-operative follow-up, and other related services.
When a patient with aortic disease requires access to a vascular surgeon.
This may involve urgent or routine surgical evaluation, pre-operative testing/work-up, explanation of different surgical procedures/options for referred diagnosis, appropriate follow-up in regards to image surveillance, post-operative follow-up, and other related services.
When a patient requires genetic counseling and/or surveillance for aortic disease.
This may include situations when an existing cardiologist, surgeon or primary care physician is already managing the patient's care and only wants genetic counseling/surveillance. If genetic testing is recommended and obtained, genetic counseling will be provided. The MI-AORTA program can also facilitate evaluation by a Michigan Medicine provider if desired by the referring physician.
When a patient with a genetic condition that potentially predisposes them to aortic disease requires access to any Michigan Medicine Specialty.
This may encompass patients with various genetic disorders such as Marfan's syndrome, Vascular Ehlers-Danlos syndrome, Turner's syndrome, and Loey's-Dietz syndrome. Such patients might present with scoliosis and require a expertise of a thoracic surgeon. Additionally, they may need specialized care from ophthalmologists or reproductive endocrinologists to assist with family planning, among other related services.
If you are a patient who has a known aortic disease, a strong family history of aortic disease, genetic conditions that predispose you to aortic disease, or other related conditions.
