The Stage 1 VTRA Report Form is completed by the Social Worker or Psychologist attending. No copies are to be made of the document for any purposes and those in the meeting are encouraged to keep their own notes while ensuring that absolute confidentiality is maintained (e.g. not shared with others, put in OSR, etc)

The OSR Form must be filled out and put in the OSR the day of the Stage 1 Meeting. The only document which goes into the OSR is the OSR FORM which is printed on coloured paper and only contains specific, minimal information. Must be completed at end of Stage 1 and a copy faxed to Melanie McCauley in Psychological Services

Parents or students aged 12+ must provide consent on THIS FORM. Please have them include any and all agencies that can be involved including any agencies they are not involved with but would like to potentially become involved with to support.
