I would like to start by thanking you for all the support you have provided whilst I have worked towards completing the (Volunteering, Physical, Skill) section of the Duke of Edinburgh Award. When I began my aim was to (INSERT YOUR AIM HERE) and over the last three/six months I have learnt (WHAT HAVE YOU LEARNT).

The next part of the process will require the assessor (yourself) to complete an online assessor report to demonstrate my progress over the last three/six months. The report should include information on; 1) what progress I have made towards my goals, 2) what have I achieve, 3) how frequently did I take part in the activity.

To complete the report you will also need my eDofE number (INSERT NUMBER HERE) the section you are assessing (Volunteering, Physical, Skill) and the dates I have being taking part in the activity (INSERT DATES HERE).
