Talent Release forms:

Click on the icon of form to access a Google Forms copy of the Talent Release Form.

Every student who appears in the film must have a talent release form signed by his/her guardian or parent.

If the student is 18 years of age or older, he/she may sign the form. Forms must be received before the day of the festival.
