COVID-19 Self Reporting form

To ensure that UA&P can provide support to faculty and staff who have been exposed to or have tested positive for COVID-19, employees are requested to immediately report to their respective supervisors and fill out a COVID-19 Self Report form. HRM will coordinate with the supervisors for the monitoring of the case. All reports that will be submitted will be maintained as confidential records.
