Through the e-RCS request web portal

For reimbursement applications, you may file through the following options:
Through the Avega Agora app or
By submitting the duly accomplished Avega Reimbursement Form and requirements to avega.reimbursementfiling@avega.net.ph , CC: izabelle.paterno@uap.asia
When submitting the reimbursement request via email, please use the format below for the subject of your email:
"Filed Reimbursement-Name of Member or Patient- Company Name"

Avega Card Replacements
Employees who lost their Avega cards can download a digital Avega card through the Avega Agora app.
Employees who would like to request for a replacement of their physical cards may do so by submitting the following requirements to HRM.
Requirements:
Affidavit of loss
Proof of payment of the replacement fee (Php 100.00). Employee/ Avega Member can deposit the payment to any AVEGA Bank Accounts below:
