Formal Mentor Feedback
Feedback is crucial in order to support continued student learning. Students will be reflecting on both formal and informal feedback received from mentors during their fieldwork placement experiences.
Mentors are asked to formally complete feedback, using this NURS 425 Mentor Feedback Form, at midterm and end of term. Students have access to the word formatted document and will provide this to you via email.

Student Reflection and Goal Planning
Each month students will submit to their course professor two documents: Monthly Progress Report and Fieldwork Activity Log.
These tools will be used to track student progress and provide students with a place to reflect on their activities and informal peer feedback to develop personal learning goals. In addition to the formal midterm and final evaluations, these documents will be used to determine the adequacy of student learning and progression throughout this course.
