Fill out these two form: Form 1- Application for Working Certificate and Form 2- Application for Employment Permit
Also,
Get a signed note from the applicants doctor or a physical from the last 12 months stating you are able to work.
a COPY of the birth certificate or passport stating that they are at least 14 years of age and a COPY of your social security card.
COPIES NOT ORIGINALS
During the regular school year (September- June) you can bring these documents to Ms. Melendez and she will provide you with the working certificate.
During the Summer (July and August) you should head to your closest Family Welcome Center with these documents and a representative will provide you with the working certificate.
