To play sports at Foothill High, all students must complete a athletic clearance registration on Home Campus. Note that an athlete must be cleared in home campus to be able to participate in any athletic practice. 

All student athletes must have a sports physical completed by a medical physician prior to participating. This physical form must be filled out, signed, and stamped by a doctor and then uploaded to Home Campus

Any athlete that transfers to Foothill High School from another school needs to complete this form (even if they have not played a sport at the previous school). Once the form is completed, please return to the athletic director. 

Student athletes that transfer to Foothill and thier whole family moves, will need to fill out the VCR checklist and provide the necessary documents in order to be cleared to participate.
