Referral and Follow-Up Policy

Upon consideration of the client's needs and determination that those needs reach beyond the scope of Transitions Counseling Clinic, a referral will be given to the client. If the client is returning for additional services, a follow-up will be accomplished at their next appointment. If the client is leaving services, staff will notify the client that they will be called up to 7 business days after the initial referral for a follow-up.
