Work Samples

Student Name

The STUDENT needs to write or type both their first and last name.

Date

The date needs to be within the dates of the LP. Weekend and holiday dates are not acceptable.

The date needs to be in the same format as the name (if the name is handwritten, the date must be handwritten in the same pen/pencil; if typed, both must be typed.)

Questions AND Answers

Reviewers have to understand what the student is being asked to do. This is why a sample MUST include both the questions AND the answers.

Viewable

Sometimes light pencil doesn’t show through when it is scanned and uploaded. If this is the case, you will either have to use a pen or marker to trace everything or upload a different sample altogether.

** Also, if students are accustomed to writing their name on the top corner of their work, make sure it isn’t cut off when you scan and upload it.

Legible

Make sure handwriting is able to be read, especially the name and date. If there are any erasure marks on the name or date, please have the student write their initials next to the correction.

If there is more than one page for the sample assignment, please include all of the pages.

Submit as PDF

Please make sure you submit your work samples in PDF format.
