ACCREDITED ASSESSMENT CENTERS
Accredited Competency Assessment Center (AC) is an establishment officially authorized by TESDA to manage the assessment activities of candidates for national Certification (PM Accreditation Of Competency Assessment Centers 2017).
It is where the National Assessment for Certification is conducted.
There are accredited Competency Assessments Centers in the whole Philippines for different qualifications. For the province of Camiguin there are only three(3) Accredited Assessment Centers as of April 15, 2019 with Twenty One (21) qualifications in Nine (9) sectors, as shown in the Registry of Accredited Assessment Centers below:
Steps in the application for Accreditation of Assessment Centers
1. Visit the TESDA Provincial Office for the Conduct of Orientation and get the Certificate of Concurrence.
2. After issuance of Certificate of Concurrence
3. Checklist of Requirements to be submitted after the conduct of Orientation:
1. Letter of Intent
2. Copy of SEC Registration or equivalent (CDA- registered, R.A., except Sole Proprietorship)
3. Financial Statement (Latest audited)
· For New Company: Paid up capital required by the SEC
· For Existing: Latest Audited by a third party
4. Business Permit (Current and valid)
5. Fire Safety Certificate (Current and valid)
6. BIR Registration (Valid)
7. Company Profile
8. Organizational Structure
9. Staff Complement and Profile
10. Building lay-out/floor plan/shop lay-out
11. Self-Assessment Checklist (TESDA-OP-CO-03-F02)
12. List of complete facilities, tools, equipment, and materials appropriate to the qualification/ applied for (identified in the CATs)
13. Location map
14. Lease Contract/Proof of Ownership of the location/premises of the Assessment Center
15. Checklist of tools, equipment, supplies and materials, and facilities (TESDA-OP-CO-03-F04)
4. Wait for the result of evaluation of application documents.
5. A Letter of Notification will be issued on the result of evaluation of your application.
6. If compliant, you will be notified for the schedule of site inspection and require to pay the 50% Accreditation fee prior to Site Inspection.
7. If the result of Site Inspection is compliant, the Provincial Director informs the Applicant-AC of the approval of the application and provides the Affidavit of Undertaking (AOU) (TESDA-OP-CO-03-F10) one AOU per qualification to be signed by the AC Manager, for notorial, and require to pay the 50% balance, Accreditation fee.
8. PD trains the AC Manager and Processing Officer in the following prior to the operation of AC:
· On the use of the T2MIS and AC Operations
· The PO CAC focal provides the Accredited AC copy of the T2MIS Users Manual and AC Operations Manual
· The Provincial Director issues a certificate of Training (TESDA-OP-CO-03-F11) to AC Managers and Processing Officers
9. After receipt of the Notarized AOU, you will be issued with the Certificate of Accreditation.
