Training and assessment

To register please fill up this online form or proceed to the "Application Form" tab of this site.

We will be scheduling you to a class/assessment and will contact you once there is a schedule available. 

Please bear in mind that scheduling will follow a first come, first served basis and may take time if the qualification has a lot of applicants.

REQUIREMENTS:

For online TRAINING AND ASSESSMENT registration:

Submit the following requirements AFTER REGISTRATION:

        1. Colored ID Pictures, white background (Acid base, not computer printed) with collar

      - Passport size- 4 pcs (white background w/ collar|with name and signature at bottom; CSC format)

      - 1x1 size          - 4 pcs

        2. Birth Certificate (NSO), clear copy

        3. High School/College Diploma or TOR, clear copy

        4. NCAE/NSAT/NCEA Result for High School graduates (if applicable)

5. Working in a Gender-Diverse Environment certificate (https://e-tesda.gov.ph/course/view.php?id=2497)

        6. White Long Folder with fastener (put all documents inside folder)

        7. (For TM1) College Diploma and National Certificate (NC) II of any qualification

*For those without NCAE, you may take the power tests (TLT, OLT and TNT) using the links below:

        ** Note that a valid email address is required upon application to ensure that there will be no issues during training.

Submit the following requirements ONCE THERE IS A CONFIRMED SCHEDULE:

        1. Medical Certificate (from Government doctor/health facility), clear copy

        2. Barangay Clearance, clear photocopy

        3. Student Permit or Driver’s License (for Driving NC II applicant)

Once complete, submit the requirements to TESDA PTC Carmen and wait for our message on your schedule.

For online ASSESSMENT only registration:

Submit the following requirements AFTER REGISTRATION (failure to do so will mean that your application is not yet complete):

        1. Application Form (To be printed in PTC Carmen if pdf file is not requested and to be signed by the candidate)

        2. Self Assessment Guide (To be printed in PTC Carmen if pdf file is not requested and to be signed by the candidate)

        3. 3pcs. Colored, passport sized photo (white background w/ collar|with name and signature at bottom; CSC format)

        4. Certificate of Employment

        5. Photocopy of Birth Certificate (and Marriage contract for married female applicants)

        6. Certificate of highest educational attainment (HS or college diploma)

7. Photocopy of 2 Government Issued Valid IDs

        8. (For Driving) Photocopy of Driver’s License or Student Permit

* Secure the requirements stated above and submit to PTC Carmen (Requirements 3 to 7).

                   > Driving NCII - 819.00php (use own vehicle during assessment) (as of Sept 6, 2021 Circular #072 s.2021)

• All applicants for competency assessment, upon submission of their application requirements, shall pay the processing fee amounting to P35.00php to TESDA PO/DO on top of the Assessment Fee stated above. [TESDA Circular 017 s.2024]
