EMA is a web-based application which allows Parents/Guardians to complete permission forms, athletic forms, and medical information forms online

If you have transferred to York Mills from a different high school that is not a junior high feeder school you may need to fill out these transfer forms.
Please see Ms. Chan in the Double Gym office to discuss if you need to do this.

If you do not get this done you may not be eligible to participate on any sport team here at York Mills.

Get this form signed by your parents and bring it in to the office to get the principal (L. Gage) to sign. We will need a copy of your insurance as well.
Once you receive the signed form from the office you must keep it in your vehicle when you are driving any other students.
