How to fill in the permission Form:

Go to https://parent.ema.tdsb.on.ca/#/
Click on Athletics and then Athletic Teams
Click on Select Student and then scroll down to the team you are signing up for.
Scroll down on the next page and click on Apply
Fill in the permission form!
