Counselor Letter of Recommendation Request Form: Senior Profile (Brag) Sheet

If you will need a letter of recommendation from your school counselor, you must:

1. Complete the attached form. 

2. Schedule an appointment with your counselor (through email).

3. Bring the completed form with you to your appointment with your counselor.

4. The completed form must be turned in at least three weeks in advance to ensure your letter will be completed by the due date. 

NOTE: DEADLINES ARE THE STUDENT'S RESPONSIBILITY. 

YOU WILL CLICK THE LINK BELOW, CLICK MAKE A COPY,  TYPE YOUR INFORMATION INTO THE GOOGLE DOC, SHARE THE GOOGLE DOC WITH YOUR COUNSELOR, AND ALSO PRINT A HARD COPY OF THE LETTER OF RECOMMENDATION, AND DROP IT OFF IN THE COUNSELOR'S OFFICE.
