FAQ: Transition Services
What are transition services?
Section 5 of the student’s IEP outlines post secondary outcomes which may include education, vocational training, employment, volunteering, community based experiences, and independent living.
The objective is to provide a coordinated set of transition activities that work toward a student’s post-secondary outcome.
Targeted instruction is provided for those area of needs that are barriers to each student’s outcome.
Transition planning is about preparing each individual student for life after high school.
Transition services are a coordinated set of activities and experiences for students starting at age 14
Transition services should be individualized based upon students individual needs and align with each students post-secondary goals
What are the different types of transition services?
In-school work-study
Community based volunteering
Community based work-study
Community experiences
In-School Work Study
Career Lab:
Career Lab is a hands-on job training and assessment program.
Individual In-School Work Experiences:
Students work alongside a job coach and/or peer models in a variety of in-school environments to address their job skill development needs.
Jobs include the cafeteria, office, mail sorting/delivery and bookstore.
Community Access Experiences
Students will learn to access their community as a patron alongside a community coach or educational assistant.
The students will participate in community based instruction in order to address behavioral and social needs that have been identified as barriers to the students' ability to fully participate in community based living.
Students may access locations such as:
grocery store
REC Center
Library
Community Based Work Study
Students work in small groups with a job coach within an authentic, community based work site
Job sites vary year to year and specific job training placements are determined by each students needs
Placements are an IEP team decision
Community Volunteering
Students will access local non-profits to learn the benefits of giving back to their community.
Volunteering locations include:
