If your student will be absent 3-15 school days please email ryan.vannostrand@sweetwaterschools.org with the following indormation:

1. Student Name

2. Student ID #

3. Reason for Contract

4. First Date of Absence

5. Last Date of Absence 

6. Return to School  Date

The purpose of this Independent Study Contract is to excuse and document student absences during the approved period, allowing the student to receive academic credit for those days.

All assignments, coursework, and instructional materials associated with the contract are the responsibility of the student’s teachers. Communication regarding assignments, expectations, and completed work must take place directly between the student and their teachers throughout the duration of the independent study.

This contract serves solely as official documentation to clear the student’s absences. All academic responsibilities and related communication are to be managed between the student and the teacher.
