DIAA Athletic Physical and Consent Forms

The DIAA Sport Medicine Advisory Committee recommends that the required forms be completed by the student athlete's primary care provider to ensure continuity of medical care. 

These forms must be completed AFTER April 1st each year based on a physical performed by the signing physician within one year of the date of signature.

All student-athletes must register and upload their DIAA physical form online through Snap Manage.

Important: If you do not receive a confirmation email from Snap Manage, your registration is not complete, and your athlete will not appear in our system.

You should register for Fall sports NOW, but you can also register for Winter and Spring Sports at the same time.

Registration Requirements

1. Complete the Snap Manage registration.

2. Upload a current DIAA physical (2025–2026 form only).

3. Log back in 1–3 days later to confirm your physical has been marked “Approved.”

   • If it does not say “Approved,” it is incomplete and your athlete is not cleared.

Helpful Tips for Parents

• Medical Alerts: Add anything important for coaches to know in the medical alert section.

• No confirmation email = not finished. Log back in and make sure you selected a sport.

• Upload issues:

  • Many parents find it easier to upload using a computer instead of a phone.

  • If using an iPhone, scan the pages in Notes or turn photos into a PDF before uploading.

  • Avoid uploading single photos – Snap requires a PDF of the full form.

  • If the file is too large, save/print as “Microsoft Print to PDF” in black & white to reduce size.
