• Physical examinations are required by the state upon entry into the school district. Physicals must be completed within 365 days of the student's first day. 

• Immunization records should be kept current and a copy should be provided to the school nurse upon enrollment and after new immunizations are received.

• Students who transfer from a country that has a high incidence of TB must have a TB test for purposes of enrollment. 

Students who are transferring from within the State of NJ must submit all forms before their first day of school.

Students who are transferring from another country or another state have 30 days to submit physical, immunization and TB paperwork. 

• The American Academy of Pediatrics recommends annual physical exams. The New Jersey Department of Health maintains that physical exams should be performed at least one time during each development stage: early childhood, pre-adolescence and adolescence.
