Physical examinations are required by the state upon entry into the school district. Physicals must be complete within 365 days of the student's first day.
Immunization records should be kept current and a copy should be provided to the school nurse upon enrollment and after new immunizations are received.
Annual physical examinations are required for participation in high school sports. Forms can be found on the district's athletic webpage at
new 2024 available soon.
New forms will be required as of 7/1/2024.
The American Academy of Pediatrics recommends annual physical exams. The New Jersey Department of Health maintains that physical exams should be performed at least one time during each development stage: early childhood, pre-adolescence and adolescence.
Physical Education excuses due to a medical reasons need to be provided in written form.
A note from a parent or guardian can excuse a student for up to 3 days.
After 3 days a physician's note is required for your child to be excused from Physical Education Classes.

Students who need a new physical exam: please take form 1, form 2, and form 3 to your doctor. The doctor will complete these forms and keep form 1 and form 2 in the student's medical file at their office. The doctor must complete form 3 and the student must return that form to the athletic trainers for processing.
Physical exams are good for 365 days before they expire. Exams must be valid in order to participate in tryouts. Students with expired exams must submit a new exam prior to participating in sports.
Students who have existing physical exams on file only need a Health History Update Questionnaire. Please have a parent/guardian complete this form, sign it and return it to the nurse or athletic trainer.
Estudiantes que necesitan un nuevo examen físico: por favor lleven el formulario 1, el formulario 2 y el formulario 3 a su médico. El médico completará estos formularios y guardará el formulario 1 y el formulario 2 en el expediente médico del estudiante en su consultorio. El médico debe completar el formulario 3 y el estudiante debe devolver ese formulario a los entrenadores de atletismo para su procesamiento.
