Application  for Induction into National Honor Society

Application Process Directions:

**Once you have been notified that you qualify to apply to ONW's National Honor Society, you can start the process of applying.  

To be considered for NHS, you must fill out all parts of the application completely and submit to Mrs. Pruett-Epp (1304) or Mrs. Uden (2221) by November 14th at 3:00 pm. 

The links will go live once candidates have been notified about potential membership with the exception of recommendation links and the link to log hours which will not go live until after the candidate meeting. Your name will be added to the log's drop down menu once we have received your completed application.

• ​​​​​Student Contract: Print and complete.  Both student and parent/guardian must sign and date to be accepted.

• Student Activity Form: Print and complete.

• Substance Abuse Form: Print and complete.

• Recommendation Forms (School, Community, 3rd Sponsor): Provide the link to your sponsor/supervisor to complete.

• Service Hours Logging Form (not due until Feb 20): Below

References: All three reference forms must be submitted by the application deadline date. One form must be from a school activity (a coach, sponsor of a club, etc), one reference must be from a community activity (youth group leader, boss from job, coach of club team), and the third form may be another school or community activity (can be either but not a teacher -unless they are a coach/sponsor). 

The purpose of these three recommendations is to verify that you are involved inside and outside of school in activities and/or service.

You can check with Mrs. Pruett-Epp or Mrs. Uden to verify if the forms have been turned in. Late forms will not be accepted.  It is your responsibility to remind references to fill them out and get confirmation from them that it is completed. The link to send to your references will be active in October. It is suggested you contact each sponsor to ask them to recommend you in advance, then send them an email with the link to click and complete.

Service Hours: As a candidate, you must log a total of 20 hours of community service.  Candidate hours must be completed between October 1 and February 20 (3pm). 15 hours must be of service to the community and 5 hours must be of service to the school. The service hours link will be located on this page and will be uploaded in October.

Completion of the forms and service hours does NOT guarantee that you will be inducted into NHS. This will allow you to be considered for induction. You will be notified whether or not you are accepted after the Faculty Council has reviewed applications. Induction into ONW's National Honor Society occurs in April.

You are considered a 'Candidate' until you are inducted. Then, you become a 'Member'. 

For those who did not attend the informational meeting, before your name will appear in drop down menus for recommendations or logging service hours, you must first show an interest by completing the contract and activity forms and returning them to one of the sponsors.
