Where do I begin?

1. Become enrolled in Barron County's CCS program.

2. Request a referral to the CCS provider: "Strive Wellness"

Please indicate if you have a Strive Wellness CCS provider preference or if you are open to either provider. See below for provider spotlights.

3. If we have availability you should hear from us within 2 weeks after we receive the referral. If we do not have availability, we will inform the service facilitator and offer you to be placed on our waitlist.

4. Waitlist times are highly variable. Typically we observe services to last 1-2+ years with each family. We value developing a strong, genuine, and reciprocal working relationship where you can show up with YOUR WHOLE SELF.

What to include (CCS Service Facilitators):

1. Signed ROI from Barron County including the family's signature to Strive Wellness.

2. Most recent CCS Assessment summary.

3. Current ISP.

4. Please indicate if the referral is open to anyone within Strive Wellness or for a specific provider.

5. 1-2 paragraphs outlining services being requested, difficulties family is experiencing, and what the general goal for Strive Wellness to work towards.
