1) When a denial is received due to "The MDS was not submitted to the national repository", is it possible to correct this error with late submission of the MDS? The denied dates of service are from 2021. Meaning if the ARD was within the last two years, could it be submitted now (late), even if it was never submitted in the first place? 

2) A provider has a resident with a Medicaid card that shows one date of birth, while the resident's driver's license shows a different date of birth. The current RAI Manual does not indicate what document(s) to use to verify the correct resident's birth date. How should the date of birth be verified? 

3)  When I am looking at the list of residents triggering on the publicly reported quality measures and it says "residents who were not assessed and appropriately given the seasonal influenza vaccine." What does that mean exactly?

4) Is TPN considered IVF? 

5) The following three (3) questions are about resuming Medicare following hospitalization or other circumstances:

A) Assuming the following is based on a new issue and not an interrupted stay, would it be correct that if a resident's 5-day PPS was completed, was discharged from Medicare, but had days remaining, s/he would need to meet the qualifying stay (3 days in hospital) and 60-day wellness period to be picked back up?  

B) If a resident had the same skilled need, no interrupted stay, and a discharge PPS was completed and had days remaining, can s/he be picked back up on Medicare by doing a new 5-day PPS, or does the qualifying stay and wellness period apply no matter what, outside of an interrupted stay.

C) If a provider discovered that a PPS discharge should have been completed but wasn't and it is past the 100 days, the ARD cannot be backdated, should it be skipped or set the PPS D/C ARD for the day identified and perform the PPS D/C? 

6) Now that the items for the PHQ 2-9 are part of the data elements for the SNF QRP APU, what is the process if a Medicare resident discharges unexpectedly and the interview has not been completed?
