Computer Use Agreement/Insurance form

• Students will be issued a Chromebook after completing the Computer Use Agreement/Insurance form.

Immunizations

• Updated grade 7 immunizations are due to the nurse by the first day of school. Please notify the nurse if there is a reason for delay. All mandated forms and nurse information is available at on the district website. Click the link below!

Mobile Dentist

• Grade 7 mandated dental exams done over the summer can be forwarded to the nurse. If you choose to see the mobile dentist in October please sign up by clicking link below.
