Accidents and Injuries

A student’s health, safety and wellness is a priority and main concern at CLS. Our goal is to support a healthy learning environment for all. Accidents and injuries that occur during school hours will be attended to by the Health Office staff and the CLS Emergency Team if necessary. Incidents that happen after school hours or during sporting events will be handled by the supervising staff member or team coach. 911 will be contacted in the event of an emergency. To help our Health Team provide optimum care for students, it is mandatory to complete all  medical forms yearly including updating MyCLS with all medical conditions and emergency contact phone numbers.

Health Care

Students with medical conditions such as asthma, food allergies, diabetes, and epilepsy need to have additional paperwork on file that is updated every school year.  Students are not allowed to self-carry medications, however, rescue medication (EpiPens, Inhalers, Glucagon) can be self-carried with a medical provider and parent's permission.  To access and download our CLS Medical forms and Sports Physical form, please see the Resource tab on MyCLS.  Please upload all medical forms yearly in MyCLS.  All Sports Physical forms must be turned in to the Athletic Office.

Student Illness

If a student becomes ill at school, a pass will be provided to the Health Office. Students leaving school with an illness or minor injury must sign out in the Middle/High School Office and may only leave the school with a parent or parent-authorized contact person.  Health conditions that require a minimum 24-hour stay at home include:  a fever of 100.4 or greater, diarrhea, or vomiting.  If a student is unable to return to class or requests a visit to the Health Office multiple times within a school day, a parent will be contacted to pick the student up from school.  If it is determined that your student needs to be sent home from school, your student must be picked up within 60 minutes.
