The University Interscholastic League (UIL) requires that every athlete receives a physical from a doctor and a completion of medical history by the guardian. S&S CISD honors physicals for 2 years, therefore, every 2 years our S&S CISD athletes must get a physical in order to participate in any sport.  

Print the form out, fill out the guardian portion including medical history, the physical examination must me completed by a medical doctor: 

Rank One is a website that S&S CISD uses to complete UIL/S&S athletic administrative paperwork. You will create an account, find your child's information and digitally sign the documents. 

These documents must be signed each year prior to athlete participation.
