Eligibility Requirements for Business Owners/Operators:

• Rebuilding establishment and/or opening a new one - must provide documentation with any updates on your reopening process.

• Must have been in business for at least 12 months before Disaster (June 2024 to June 2025).

Documentation Requirements for Business Owners/Operators:

• Pay stubs and/or any draws taken over the last twelve months. 

  • If you directly transfer funds, please provide copies of your banking documents. 

• Employer Identification Number (EIN) letter. 

• Profit and Loss statements for 2024, and 2025 if available. 

• 2024 tax filing (or the most recent one).

• Business lease agreement. 

• Food handler’s license. 

• Proof of insurance.
