Instructions for working your patient list:
Always work from the most current patient list received. Disregard last month’s list.
If you can't reach a patient with the phone number provided on the spreadsheet, try the phone number attached to their Pionneer profile or check prescription images for a phone number.
Please keep notes in the "comments" cell to track attempted calls or other information that can be helpful for the next person working on HRA's.
Use the Sona HRA Form linked below. This form sends to Sam before official submission. If there should be anything changed or corrected after completion, please let Sam know.
If a patient shows up on your list again, that means they are eligible for another HRA, and you will be paid for completing the second HRA.
Reach out to patients listed on the spreadsheet in the current month.
Let the patient know that their insurance has indicated that the patient has not had an updated A1C in the last 6 months and they are elligible for a free A1C test and diabetes counseling at Sona Pharmacy located at 805 Fairview Road Asheville, NC 28803.
Make sure to schedule a time when a student and certified pharmacists are available at that location to conduct the test.
