You must fill out a Field Trip Permission Form whenever we go off-island for an event or performance. 

The Health and Consent Form is for Thespian Events. All participating students must bring two copies with them to the registration table. One will be kept on their person, and one will be on file with the event organizers in case of emergency.

The Co-Curricular Policy and Signature Page must be reviewed and signed by each student before being allowed to participate in FHHS/MS shows or becoming a member of ITS.
