Rates and Insurance

Maintaining client confidentiality and treatment integrity is important to me. Clients frequently choose to pay out-of-pocket, as insurance companies require that I assign a diagnosis in order to receive coverage. I do not believe that every person who might benefit from counseling has a diagnosable illness. Out-of-pocket payment also allows for us to decide together the type of service and time frame that best suits your needs, rather than the insurance company making those decisions.

Should you decide that you wish to use insurance, I am affiliated with the following providers:

    • ACI Specialty EAP Benefits

    • Anthem Blue Cross/Blue Shield

    • Beacon EAP

    • CareSource: Marketplace, only

    • Chestnut Global Partners International (CGP)

    • IMPACT EAP & Work/Life Program

    • Medical Mutual

    • Medicare

    • Mutual of Omaha EAP

    • OSU Healthplan (Faculty, Staff, and Students)

    • UnitedHealthcare

    • Value Options

    • Wellpoint EAP

    • Workplace Options EAP

Payment

All out-of-pocket payments (i.e., deductible, copay, coinsurance) are due at the time of service. Cash, check, or CashApp are preferred, as credit cards are assessed an additional 4% service fee. Checks returned for Non-Sufficient Funds (NSF) are subject to a $30 service fee plus an $8 bank fee. If your account goes to collections after 90 days of non-payment, all applicable fees will be added to your balance.

Cancellation Policy

My cancellation policy requires that you give 24-hours notice of cancellation. Missed appointments are billed at the full service/session fee; no exceptions.

Note: Unfortunately, I am unable to submit insurance claims for missed appointments.
