The priorities for FY 2013 are as follows: The overarching priority is to increase physician embracement of priorities and to work with them to bring efficiency and knowledge to create excellence with the topics outlined.
1. AMI with focus on PCI time and statin at discharge;
2. Stroke Care with focus statins at discharge, getting patients assessed for Rehab Services;
3. Sepsis Management with focus on all indicators;
4. CHF with focus on discharge instructions(medication reconciliation) and readmissions;
5. SIP with focus on 24 hr. discontinuation of antibiotics, timely catheter remove, appropriate glucose levels for cardiac surgery patients, use of Beta Blockers for appropriate individual and the two DVT indicators;
6. Glucose control with focus on basal insulin; discontinuing drips on the floor; this includes the mgmt. of glucose levels for CABG patients;
7. DVT with focus on all indicators;
8. CAP with focus on blood cultures drawn before antibiotics given and appropriate antibiotics given;
9. Customer Service;
10. Patient Safety with focus on reduction of falls and falls with injury.
