CERTIFICATION OF MEETING UNION HOSPITAL HEALTH GROUP'S POLICY REQUIREMENTS FOR EDUCATIONAL INSTITUTIONS

Union Hospital Health Group (UHHG) refers to Union Hospital, Terre Haute, Indiana and Union Hospital, Clinton, Indiana.

*This Certification Form must be completed and sent to the Volunteer Services Department at Union Hospital or Administration at Union Hospital, Clinton before any student can participate in a clinical rotation. UHHG reserves the right to refuse a student a clinical experience without the "Institutions" completed Certification Form. Certification forms need to be submitted at least one (1) week prior to the start date of any clinical rotation.
