Our mission is to provide the best, most comprehensive, and continuous health care from birth to the end of life.

We will facilitate access into the health care system and coordinate the overall medical needs of each patient.

• We participate with Blue Cross/Blue Shield, Independent Health, Univera, Empire Plan, United Healthcare, Medicare, Medicaid, and many private insurance plans.

  • We are affiliated with Brooks TLC Hospital System.

• We do offer reduced fees, using the sliding fee scale. Request application from front office staff.

Video Visits: All visits for patients with any COVID symptom(s) will be done virtually via a video visit. You must have an active patient portal account for a video visit. Call the office for assistance with portal setup.

After Hours: for urgent needs or concerns after hours that can not wait until the next business day, our answering service number is 716-257-1203. If you have a true medical emergency, go to the nearest emergency room.

Mobile Mammography:

The Western NY Breast Health/ECMC Foundation mobile mammography unit will be on site on Tuesday, May 4, 2022. Call 632-7465 (option #3) to schedule.

Windsong 3D mobile mammography unit will be on site on Thursday, April 7, 2022 from 8 am to 4 pm. Call 929-9494 to schedule.
