Our Participants 

Each program participant is unique and we work tirelessly to address their individual needs. Our Care Managers/Housing Navigators provide guidance and support, and connect each participant to services to help each person attain health, well-being, and independence. The success we achieve, large and small prove all the hard work is worthwhile.
