Please select the application form from below & print off and complete. and return by post to the address given on the form.
Please select the form you require from below, by clicking on the 'pop out' icon in the top right hand corner of the form, which should open the form in a new web page for you to print off.
The first row is the Junior & Youth application in a PDF format
The second row is for the adult application form in a PDF format
The third row is the Adult standing order form in PDF format.
You can open a document by clicking on the box in the top right hand corner of the form.
Please Note: there is currently a waiting list to join the Junior or Youth groups. Please email SADS first to check on availability within the groups before completing any application forms.
